A U.S. Utility Patent Application Transmittal Form (Form PTO/SB-05) or a transmittal letter is a “cover” letter filed with a patent application listing all the papers (e.g., specification, claims, drawings, declaration, information disclosure statement) filed as part of the application. This form also lists the applicant’s pedigree information and basic filing information such as the type of application, the invention’s title, the application’s contents, and any enclosures.
